Confidentiality & Privacy Policy

The law protects the relationship between patient and doctor, and information cannot be disclosed without written permission.

Exceptions include:

  • Suspected child abuse or dependent adult or elder abuse, for which we are required by law to report immediately to the appropriate authorities.
  • Threats of bodily harm to another person or persons.
  • If a patient intends to harm himself or herself, we will make every effort to enlist their cooperation in ensuring their safety. If they cannot cooperate, we are obligated by law to take further measures without their permission, as safety is paramount. 

Fee Policy

Please contact our administrative assistant or your individual clinician in order to obtain information about our fees.

In order to provide you with the maximum control of your records and privacy as well as freedom of choice in selecting the professionals with whom you work, CAPS clinicians are not participants on any insurance plans. We will provide you with an invoice for the services that you receive, which can be submitted to your insurance company for reimbursement. We are considered “out-of-network” providers, and your insurance will reimburse you at a rate based on your particular plan. Of course, we cannot guarantee that your insurance company will reimburse you for services, as plans vary considerably. We recommend that you contact your insurance provider to discuss the benefits of your specific plan.
